A 73 year old woman presents witha 2.5 cm firm thyroid nodule, with no palpable neck nodes. She is asymptomatic. FNA is described as "suspicious for papillary carcinoma". She adamantly refuses any surgical treatment of any kind. What is the best second line treatment? I expect little would be accomplished with I-131 in view of the bulk of thyroid tissue. Besides intense thyroxine suppression, what else is there?

How about doing nothing? The survivial of these patients approaches that of the general population.

I would have thought that an increased dose of I131 would suffice - as used when only one lobe has been removed in follicular Ca. Other option would be external beam irradiation.
