Incarcerated, perforated inguinal hernia - Ärzteforum

60-year-old male presents with an incarcerated right inguinal hernia. The patient is taken urgently to the operating room with the patient is found to have a knuckle of ischemic bowel in the hernia. The bowel was clearly perforated. What is the best repair performed under the circumstances?

Bowel resection and anastomoses. Wouldn't close the perf imho.

Tissue repair and cover with vac dressing - can do later mesh once healed.

Resect, anastamose either through hernia or if necessary an accessory incision, Bassini/McVay, skin open with potential delayed primary closure

Plan on fixing hernia with mesh when it recurs. Which it will.

Yep, kind of what I was thinking. Scanned the literature and there really isn't much there.

Bowel resection and repair hernia with riabbsorbabile sutures. If hernia recurs then you may use mesh.

I have done Lichtenstein repair after resection couple of times and have got away with it.Prolene handles infection very well and gets incorporated even in presence of high bacterial load.Preferably light weight Prolene should be used.
