what does carefully mean, is it that only careless surgeons have complications !!! I hope all surgeons are careful all the time not only when taking an extra risk.

I never suggested starting with an open chole but only being ready to convert without hesitation. As far as I am concerned the additional risks without controled insertion of trocars under direct vision as has been discussed in this forum are unacceptable. Whats wrong with this or what's so beneficial about 'careful placement of the veress neeedle' against 'direct vision insertion.'
